Painting on canvas

I found this piece, looks likes it's possibly oil paint on canvas. I took pictures of all the markings on it. The pencil on the wood is written on the back of the frame. The signature is in the bottom right corner, but I can't seem to find an artist by that name. Any info would help. Thank you!

 

Hazelleaf? Very unusual name. Probably a hobbyist painter or possibly an artist whose "work" was only known locally to where they lived. 1965. The pencil notes on the back are from the framer and mean nothing to identifying the artist or locale.
