I’m not quite sure what this is, Google translate hasn’t been very helpful. Is this some sort of an intake document for the camp mentioned?
My 16yo father was a member of the Szare Szaregi (Grey Ranks) and participated in the Warsaw Uprising of 1944 as a “sewer rat”, ending up in Stalag XI-B afterwards.
My father passed in 2004. My mother is still with us but she does not remember what happened to the original of this document, all we have is this photo copy.
    Thanks!

And for anyone interested, this is a (rather poor) copy of an identity card issued to him after the camp was liberated in April 1945. Again, we unfortunately can’t locate the original.

Interestingly, and assuming they went in a logical sequence of numbers (and assuming the 141004 is the ID of the person), this person was not too far behind your father:

the first one is the transfer of 2000 - unclear if marks or zlotys - that were taken from your father and transfered to the next Stammlager he was held prisoner.
the second one is a card to facilitate the US authorities the identification and was normally ascertaining the Polish nationality by a Polish officer that was present in the camp. this was done to filter out eventual Nazi patry members that still had Polish nationality due coming from parts like Silesia a.o. where many had double citizenship Polish and German.
