Maybe a pillow for a doll? I used to make similar for my dolls when I was a kid - often with blankets to match!

Not a pin cushion then.....At least not a Victorian one.

    Possibly a homemade pin cushion.

At a recent craft show there was a fabric crafter selling cat-toys that looked like this, and about this size. Stuffed with catnip. I'm sure your cat would be happy to tell you whether that is the case......the one we bought looked cute, for about 5 minutes until the cat chewed it to shreds.
I've also seen similar, stuffed with various aromatic herbs, such as lavender or potpourri; put in with clothes to make them smell nice.

It could be; I made similiar ones for straight pins. The fabric is probably 30s, but the pillow could have been made any time. I'm still finding bits of it in stashes and sending it to a friend who loves old fabric. Quilt squares turn up too.

My thought was balsam, too. My mother had them in her bureau drawers.
