My thoughts exactly......looks Italian as all getout to me....some-one wants to ward off evil spirits with the horns and wants all the good luck they can get!!!!!!!! BUT, still pretty!!!! And, yeah, what's with the skeleton hand.....an extra kink????
The skeleton fits with the 13 charm. I think it's an anti-bad luck necklace. The horseshoe is upside down too.

Those are neat. I have never seen anything like that before where the charms are arranged like that. Yep, that is bad luck to have the horse shoe upside down. At least to me the superstitious gal.
There's actually some debate over the proper way to hang a horseshoe. Some believe they must be hung with the prongs up "so the luck doesn't run out". Others believe they must be hung with the prongs down "so good luck spills down on those who pass beneath it", (hence the tradition of nailing one above a doorway.)
Hi, Growing up horseshoes were all hung upright to hold the luck. NEVER heard of pouring good luck out. It was the same as statues of elephants, if trucks were up good luck, trunks down bad luck. Different areas had different ideas. greg
