Featured Japanese silver relief plaque, anyone here read Japanese?

    Found this silver relief plaque today, wonder if anyone can translate the silver tag and possible signature on the plaque. The symbol at the top apparently represents the Yamanashi Prefecture.

    Many thanks, so grateful for your help!

    Hisashi Amano the governor died in 1968 and the Yamanashi symbol was incorporated in 1966. So I have the date pinned down to within 3 years.
