    Sweet piece! I agree 20th century because of the upturned nose. It is marked for silver and I wonder if the surrounding stones are garnets, you would have to have it tested to make sure.

    Please be patient for @Bronwen to check in, she may not be immediately available but her wealth of knowledge on cameos is worth the wait!

    Looks like drilled tumble polished garnets wired together.

    It was made in Naples, Italy, where a lot of cameo jewellery is from. The number of the maker is quite high, so I guess it is quite recent.

    The necklace copies an earlier style, ca 1890-1900, which is also when those wired gemstone surrounds were popular. I can see why they revived the style, it is a very nice and elegant design.

    Thank you. I researched the stamp and found this;

    The Laws 30 January 1968 (Legge 30 gennaio 1968 n. 46) and 30 December 1970 n. 1496 (D.P.R. 30 Dicembre 1970 n. 1496) introduced new hallmarks for silver and precious metals. The mark was modified to a polygonal shape and a star was added on the left. Numbers and Province letter were maintained.

    So it’s after 1968-70 ?

    I think that is the person who made it. He or she would work for the jeweller whose number code is next to the star. NA means Napoli, Naples.
    The cameos would most likely have been made in another workshop, specialized in shell carving.

    Yes, you are correct.

    You've been given good info by all your respondents. The one does appear to be signed, which is unusual for a cameo like that because, as you say, they are very basic work, & ubiquitous. Afraid I can't help identify the cutter. The piece is more interesting as a whole. The info you found concerning Italian hallmarks is very useful, thanks for posting it. Older cameos that were set in Italy, if they are marked at all, are usually marked for 800 silver, not sterling. Vintage, not antique.
