    It's just a name and address, in both Japanese and English. Not necessarily the artist's.

    There's no date there. The numbers are part of a standard Japanese address: big block # -- small block # -- house #. Numbers aren't assigned consecutively -- house 57 might be between house 5 and 11, for example -- so you need a map to find a location. These maps are posted on streets throughout Japan.
